If they are trying to call you, they think you're a beard/fake account. Contact them and offer a time to set up a live skype conference. If you're legit and you do whatever they ask for ID confirmation, you'll get paid. If you're a beard, I have no idea what will happen.
Never send or receive BTC from between Coinbase (or any other exchange) and a Sportsbook. Get a wallet. Send to wallet, and from wallet to your exchange.

If it were just 1 account, they would put it on delay and pay him. If it's 1 of 50 linked accounts that keep getting created to circumvent delays, I'd expect them to act more harshly.
It's easier to multi-account and bonus abuse. If you need a clean (non VPN) IP for every new bonus cheat account, you have to have a physical location, not a VPN. Each account then requires an ID + location, vs a borrowed ID + VPN.
Max bet for bonus rules are silly. If Bovada wants to limit his max bet, they should do it through their software. Not buried in T&C that no one reads.
